Male Breasts
Definition -
Gynecomastia is a medical term that derives from Greek and denotes large breasts
in a male patient. The condition could affect one breast or both breasts. In most
patients no specific cause can be found. In some, development of breasts in male
patients has been linked to conditions such as overweight and use of anabolic steroids.
Various other medications have been linked to Gynecomastia. True Gynecomastia is
due to glandular breast tissue enlargement. Pseudo Gynecomastia refers to enlargement
of breasts secondary to fat accumulations, mixed Gynecomastia is attributed to accumulation
of both glandular elements and fat.
Plastic Surgery - The surgical
treatment of Gynecomastia is aimed at removing pathological breast tissue such
as tumors and restoring aesthetics of the normal male chest. The exclusion of
pathological cause of Gynecomastia requires detailed history, physical examination
and sometimes laboratory tests that could determine abnormal estrogen androgen
balance in the patient.
Gynecomastia in adolescence - Gynecomastia is a common condition
with overall incidence of over 30% in males between 10 - 16 years of age. Many developing
boys experience enlargement of breasts during this sensitive period. Incidence could
be as high as 65% at the age of 14 and it drops to 14% by the age of 16. Most cases
of Gynecomastia in adolescence are bilateral but it could affect each breast to a
different degree. Peri pubertal Gynecomastia often regresses spontaneously. 75% of
cases resolved within two years after the onset and up to 90% resolve within three
years of onset without treatment.
Gynecomastia in adults - In some adults
Gynecomastia has been attributed to increased body fat and altered estrogen/androgen
ratio. In rare occasions, Gynecomastia has been attributed to hormone secreting tumors
predominantly testicular and adrenal tumors. Male breast cancer is an extremely rare
cause for breast enlargement, usually unilateral.
